World Champions Spain made seven substitutions, the final change seeing Napoli keeper Pepe Reina come on for injured Barcelona stopper Victor Valdes in the 81st minute.
South Africa coach Gordon Igesund attempted to remonstrate with the Spanish bench at the time, but Spain coach Vincent del Bosque ignored his pleas and sent on Reina anyway.
Fifa's regulations state: In national "A" team matches, up to a maximum of six substitutes may be used.
If Fifa downgrade the match it will not carry any world ranking points.
It was one of South Africa's most famous wins.
